Our practice is in network with the following commercial insurance plans:

We are NOT in network with any Medicare/Medicaid/Medical Assistance insurance plans.

You will be asked for your insurance information when scheduling your initial appointment. If we are not in network with your insurance plan, there may be an option of using your Out-of-Network benefits. If this is the case, we will provide you with a superbill/receipt that includes all of the necessary documentation for you to submit your receipt for reimbursement through your Out-of-Network benefits. As a courtesy to you, we will do our best to determine benefits for you, but please note it is ultimately the client’s responsibility to know their benefits. Any charges incurred that are not covered by the client’s insurance plan are the responsibility of the client.

OUT-OF-NETWORK:

If our office is not in-network with your insurance plan, you will be responsible for payment in full. You may then submit your receipt to your insurance company for reimbursement in accordance with your policy. 

RATES:

Initial Intake Appointment: $160.00

45 – 55 Minute Individual Sessions: $135.00

Couples Sessions: $150.00

We accept cash, check, and all major credit cards/HAS cards for payment.

CANCELLATION POLICY:

We require 24 hours advanced notice for cancellations/rescheduled appointments. For appointments cancelled with less than 24 hours’ notice and/or no-show appointments, a fee equal to the amount of the scheduled session will be charged.
